Every student who wants to play on a school-sponsored sport team must submit a valid sport physical examination form to the nurse’s office before being allowed to participate. The documentation must include the date the physical exam was done and indicate that the student can participate in sports (and also indicate if there any restrictions to participation). Sport physicals are valid for 13 months and must be current on the first day of the sport season. If you have any questions regarding health clearance for sport participation, please contact the school nurse’s office at 362-1743.
