Sports Registration Forms
Items Needed for Sports Registration:
Complete packets are also available in the Main Office of the middle school, Main Office of the high school and the Athletic Director’s Office.
Payment of ARHS & ARMS Participation Fee is due at time of registration. Please make check payable to ARHS Athletics, include athlete’s name and sport in memo line of the check. Payment is required to try out/practice.
The following forms need to be submitted with payment:
Chemical Health Policy Agreement
Emergency Information Card - SPORTS EMERGENCY INFORMATION CARD: A seperate emergency card must be submitted for each sport. Cards cannot be transferred from one season to another.
- Please open, save to your computer and type the information on both cards.
- Print out both copies and return with your child’s Athletic Participation Form & Chemical Health Policy Agreement and his/her fee.
One Emergency Information Card will be given to the coach & the other to the Athletic Trainer
Current Sports Physical Form, should be on file with the school nurse before the first day of tryouts. Please call the nurse’s office at the high school at 362-1744 or 362-1743. Questions for middle school registrants please contact the middle school nurse at 362-1974.
