In accordance with Massachusetts General Law, students are to beimmunized as specified by the Massachusetts Department of Public Health.See http://www.mass.gov/eohhs/docs/dph/cdc/immunization/guidelines-ma-school-requirements.pdf for the latest, most complete information on immunization requirements.
Students may be exempt from immunizations for religious beliefs. Awritten notice from the parent/guardian regarding the religious beliefsmust be kept in the student health record. Exemption from immunizationsfor medical reasons will be accepted with documentation from the healthcare provider of contraindication.
